The only lipid-soluble endogenously synthesized antioxidant‚ CoQ10 (ubiquinone) has well established roles as a free radical scavenger in both mitochondrial and lipid membranes‚ as an electron carrier essential to cellular respiration and ATP production and recently has it has been shown to influence gene expression. The reduced form of CoQ10‚ ubiquinol‚ has emerged as a more effective therapy for increasing plasma levels of CoQ10 than ubiquinone due to its increased absorption and more potent antioxidant capacity. It has a remarkably greater effect per milligram of supplementation.
In addition‚ age and a number of chronic diseases appear to inhibit natural conversion of ubiquinone to ubiquinol‚ and the ratio of oxidized to reduced CoQ10 has been suggested as a biomarker for the aging process and directly correlated to glucose intolerance. In clinical trials‚ patients taking ubiquinone then switched to ubiquinol not only had significant increases in plasma CoQ10‚ surpassing typical peak levels but they also had clinical benefit such as improvements in overall heart function. Ubiquinol has a more potent anti-inflammatory effect than ubiquinone due to greater induction of peroxisome proliferators activated receptor gene expression.

Although existing evidence has not found an interaction‚ CoQ10 resembles vitamin K structurally‚ potentially interfering with the anticoagulant warfarin.
