  • While dairy foods such as fermented milk and yogurt are nutritious‚ dietary intolerance to the milk sugar lactose poses a problem for many people. According to the National Institutes of Health‚ lactose intolerance is estimated to affect 30-50 million American adults.1

    Lactose intolerance is a condition caused by an insufficient amount of lactase. Lactase is the enzyme in the small intestine that digests lactose (a dairy sugar). Normally‚ lactase breaks lactose down into galactose and glucose that can be easily absorbed into the bloodstream. When there is not enough lactase to digest the amount of lactose consumed‚ water is retained in the bowel‚ which results in bloating and diarrhea. Lactose that passes into the large intestine is fermented by bacteria. This produces carbon dioxide‚ hydrogen and methane‚ which may lead to bloating‚ cramping and gas. Certain ethnic and racial populations are more widely affected than others. Populations showing a high level of intolerance include Asians‚ African Americans and American Indians.1 The condition is least common among persons of northern European descent.

    Some causes of lactase deficiency are well known. For instance‚ certain digestive diseases and injuries to the small intestine can reduce the amount of enzymes produced. In rare cases‚ children are born without the ability to produce the enzyme lactase. For most people though‚ lactose intolerance develops with age. After about the age of two years‚ the body begins to produce less lactase. However‚ many people may not experience symptoms until they are much older.

    Symptoms of lactose intolerance include occasional bloating‚ cramps‚ gas and diarrhea. These discomforts usually occur thirty minutes to two hours after ingesting lactose containing foods. The severity of symptoms usually depends on the amount of lactose ingested and how much of the enzyme lactase remains in the intestinal tract. Other common issues include cravings and irritability‚ skin disruptions‚ sinus and ear symptoms‚ and headaches.

    Unlike a life-threatening food allergy‚ lactose intolerance does not involve the production of antibodies and is not mediated by histamine.

    Enzymes Address Lactose Intolerance
    Some people can tolerate small amounts of dairy products. Even more can tolerate lactose-free milk or eat dairy products when they take lactase enzyme supplements. As a complement to elimination diets‚ enzyme supplementation may assist and enhance the digestion of dairy-rich foods.

    The American Journal of Clinical Nutrition found that an average of 64% cannot digest milk solids. The study suggests that people claiming to be lactose intolerant may actually be intolerant to milk protein (casein) or milk fat.
